Kindergarten

 

Kindergarten students must be age 5
by September 30 of the current school year

 

Bring to Registration:

• Official State Birth Certificate

• Proof of Residence

  • picture ID: driver’s license, DMV, Military ID or other picture ID; AND

  • house contract, lease agreement, rent receipt, mortgage statement, or tax assessment; AND

  • current utility bill (within the past 30 days)

Required Before Entering Kindergarten:

• Up-to-date Record of Immunizations

• Physical Examination form signed by qualified United States licensed health care provider (within 12 months before the date of enrollment) School Entrance Health Form pdf

• Child’s Social Security Number or Waiver Request (within 90 days of enrollment)
