Everyone must have a completed VHSL physical, dated AFTER May 1, 2011, in order to tryout for any sports for the 2011-2012 school year. Students will not be allowed to participate without a physical.

NCAA Eligibility
Student Athletes, if you plan to play intercollegiate sports in college, you MUST register with the NCAA Eligibility Center. When registering you will be asked to pay a fee and have your official transcript sent to the NCAA Center. When registering for the SAT or ACT you must use the NCAA Eligibility Center code # 9999. This will ensure that your test scores go directly to the Center. Student-Athletes who do not use 9999 will need to pay to have your scores sent later in the process. The Eligibility Center CANNOT accept ACT or SAT scores from your Deep Run HS transcripts.

Parents of All Student Athletes:
As you know VHSL Sports Physicals (hyperlink http://www.vhsl.org/files/adm-physical-form-rev-3-2011.pdf) are required each new school year for players to be eligible to participate in tryouts/practices/out of season conditioning/team sports. For the new 2011-2012 school year, physicals must be completed and on file with an exam date May 1, 2011 through June 2012 to be eligible for sports participation this year.

Message from CVS:
Certain situations require a student athlete be examined by their PCP or medical specialist. In general, these are:
· Any history of chest pain or palpitations , especially with exercise
· Short of breath with exercise or easy fatigue
· Family history of premature sudden cardiac death (e.g., before age 50)
· Diabetes
· Heart Murmur
· Musculoskeletal condition being monitored by another provider
· Joint surgery
· History of significant head injury with loss of consciousness that required medical intervention
We operate on a walk-in basis. So please emphasize with your students that any of our locations can be visited for this service. Along with Nuckols & Twin Hickory, we have clinics at Lauderdale & Church and at the Willow Lawn Shoppes on Broad St. We are looking forward to serving your student athletes in the coming months!
