DVD Order Form

Available Programming

• Most HCPS-TV programs are available on DVD upon request.
• Performances and other select programs are not available for purchase due to copyright restrictions. 
• Programming requests should be limited to programs taped within the past two school years. 

Fees

• DVD copy of single HCPS-TV program:  $12.00 (includes $2.00 shipping/handling fee)
• Only one program per DVD
• Payment should be made in the form of check or money order only (made payable to HCPS/Television Services)

How to Request a Copy

• Download the DVD order form and return with payment to:

• If you cannot access the internet, mail your request and payment with following information: your name, mailing address, telephone number, title of the program you would like, the date it was originally shown or taped and, if possible, the reason for your interest in the show to the address listed above.  Requests made by HCPS employees should include school/department name and IDT/account code in addition to the information above.

• Your order will be processed and mailed within 2 weeks of receiving your order and payment.
