Welcome to the Henrico County Public School’s website on post-secondary transition. You have probably arrived here because you have some questions about what transition means and how to successfully plan for meeting post secondary goals.
Transition planning, as defined by the Virginia Department of Education, is “the process students and their families use to think about life after high school.” Realizing successful post-secondary outcomes requires collaboration among schools, community service agencies, and students and their families to ensure that students acquire the knowledge and skills to prepare them for post school employment, community living, and postsecondary education.
This website has been specially designed to assist students and their families in planning for transition. It contains information and Internet links that we hope you will find helpful in deciding how to plan for post secondary transition.
